I got a great deal and service from foam by mail . com/foam factory. I got HD36-HQ in 4 inch. Be carefull about buying custom sizes, buying a standard size and cutting it with an electric knife is much cheaper(its easy). The cover is the same way, buy standard size and just turn inside out and sew a seam, again very easy.
